Essential functions:

• Performs personal care activities that assist the patient with activities of daily living. These could include (but are not limited to):
• Personal Hygiene
• Ambulation/Transfers
• Dressing/Shaving
• Prepares meals and snacks according to instructions
• Accompany patient on errands or medical appointments
• Performs light housekeeping activities
• Medication reminders
• Assisting with the prescribed range of motion exercises
• Assisting with blood glucose monitoring and blood pressure check
• Care of bed-bound patients
• Incontinence care
• Use lifting equipment such as gait belts and Hoyer lifts
• Providing hospice supportive care

Preferred experience in:

• Hoyer lift/Gait Belt
• Personal care
• Range of Motion Exercises
• Companionship
• Light cleaning

Must haves:

• Professional, passionate, trustworthy and dependable
• Committed to making a difference
• Must have a valid driver's license with reliable, insured vehicle
• Must be reliable and have a strong attention to detail in completing assigned tasks, communication skills and documentation
• Employment contingent upon background check and drug test.

Some duties could include (but not limited to) preparing a meal, personal hygiene (bathing, dressing, toileting), light housekeeping, laundry, medication reminders, assisting with prescribed range of motion exercises, provide transportation to appointments and errands, dementia care and companionship activities such as reading, music, puzzles, going for a walk, etc.
