Position Summary/ Key Responsibilities

  • A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
  • This role is responsible for health system navigation and resource coordination, health promotion and coaching, and health education to teach or promote methods and measures that have been proven effective in avoiding illness and/or lessening its effects by acting as a mediary between community, social services, and health systems.
  • Attendance required: all staff meeting, case management meeting, CHWSD monthly huddles, CHW/outreach team meetings

  • Sits, stands, bends, lifts, and moves intermittently during working hours. Interacts with guests, staff, visitors, etc., under a variety of conditions/circumstances. May be subject to hostile and emotionally upset guests or visitors, etc. under a variety of conditions/circumstances.

Education, Certification, and Experience Requirements

  • Education :
    • LATC/ STI courses to obtain certification
  • Certifications :
    • CHWSD certification
    • CHWSD Fingerprint Approval
    • CPR, 1st Aid, Mental Health, QPR, ASIST, NARCAN, DV Survivors, Cultural Competence, Trauma Informed Care, de-escalation and self defense, CPS reporting, APS reporting, Housing Models, and Substance Use Disorder
  • Experience :
    • Lived life experience highly desired in combination with education and certifications

  • Understanding of complex systems and social service agencies
  • Ability to work with clients in a variety of socioeconomic and health conditions
  • Understanding of data quality
