Description
  • Conduct patient evaluations in both inpatient and outpatient settings, crafting individualized treatment plans tailored to their specific needs.
  • Implement effective physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function.
  • Work closely with a cohesive team that includes nurses, doctors, case managers, and rehabilitation professionals to ensure optimal patient outcomes.
  • Educate patients and their families about therapeutic goals, discharge planning, and exercises to do at home.
  • Maintain accurate documentation of evaluations, treatment plans, and patient progress in accordance with facility policies and regulations.
  • Engage in interdisciplinary discussions and care team rounds to guarantee comprehensive and coordinated patient care.
Education
  • Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution.
Certifications
  • Active state licensure as a Physical Therapy Specialist .
  • CPR certification is preferred.
  • Additional specialized certifications (e.g., in neurology or geriatrics) are welcomed but not mandatory.
Skills
  • A minimum of prior experience preferred.
  • Ability to address and treat patients with multifaceted medical conditions and mobility challenges.
  • Strong communication and collaborative skills are essential.
